The packet may be used at any time, but the entire contents of the packet must be dissolved into a single mug when it is used.
Your task is to fix 4 cups of coffee at exactly 16% strength.

I'm sure he knew that. He didn't say that the four cups would have coffee at 100% strength but that it would have 100 % of the contents of the packet. So when he ended with 64% in 4 cups of water that's 16% of the packet for each of the cups of water.
