Given that this occurs at low flow rates (answer to my question) - I have seen this effect before. Normally, the water wold indeed continue out in a cone, and hose nozzles generally do this, however under slow and carefully symmetric conditions, an air bubble will get trapped in the middle of the "fish" shape, and then surface tension will cause the water to follow the surface of the bubble and take the shape shown in the graphic.
