My number:
1. Is a 3-digit number.
2. All its digits are prime digits.
3. A number formed by its 1st 2 digits is prime.
4. So is the number formed by its last 2 digits.
5. It is the smallest number satisfying the above conditions.

Find it.

It may not have been the Ady's intent, but 353 is the smallest of the two possible primes that meet conditions 1 to 4. The other prime being 373.
353 meets condition 3 in that the first two digits, when ordered differently, form a prime (the same prime formed that meets condition 4). 
I do believe, though, 373 is the only prime number that satisfies the zeroeth condition -- that is, "Find my_(Ady's) number".
