Take one out => A matchstick is taken out of the match box.
Scratch my head => The matchstick is scratched at the side of the match box. It is now lighted up.
I am now black => When the matchstick is extinguished it becomes black. If the fire (light) has spread the entire surface, the whole body becomes black.
But once was red => The head of the matchstick was originally colored red.
Accordingly, the required answer to the riddle is a match or a matchstick.
